The following resources have been collected and categorized for you to seek out different services available to you nationally and here in the state of Indiana, ranging from government assistance programs to local individuals who have modified equipment for veterans to participate in what was formerly an enjoyable hobby. Need a service dog?
IMPORTANT INFORMATION: Once you complete the form online, print and mail to: Indy Blue Star Mothers, PO Box 269543, Indianapolis, IN 46226-9543 along with a copy of your DD214 and a copy of your VA benefit letter. Your benefit letter must contain the information about your PTSD diagnosis.
Do not contact Ultimate Canine directly unless you plan to purchase your own service dog. Indy Blue Star Mothers may need to contact you for further information to access your need.
